Planning a Funeral

If you have recently lost a loved one, please accept our deepest condolences.


Please use one of the forms below if you are requesting a funeral service at our parish. Both the forms are the same, so pick one that works best for you.


You can fill out parish form electronically and email us! (Newer computers may open the file in the lower left corner.)


You may also print the parish form and hand write the information necessary for planning a funeral service. Drop this form off at our parish office Tuesday - Friday mornings

Cemetery

Mount Calvary Catholic Cemetery is owned and operated by St. John the Evangelist Catholic Church since 1935. Catholic cemeteries are not available to the general public, but are set aside for the interment of Catholics and members of their families who are entitled to Christian burial according to the norms and discipline of the Roman Catholic Church. A Catholic cemetery seeks to communicate the image of the communion of saints, and those who request burial within a Catholic cemetery are making this profession of faith. However, out of compassion, at the request of the family our pastor may grant permission for a non-Catholic member of a parish family to be interred in our cemetery, as long as this would not be a cause of scandal to faithful Catholics.
